This form can be downloaded and printed for students wishing to complete an additional qualification with Strategix while in school. This form provides course and fee details and seeks permission from parents and/or guardians.

Fees indicated on this form are available only to individuals who previously completed a VETiS course with Strategix and are currently still enrolled in school.
