Request for Access Form

Strategix treats all requests to release information seriously as you have rights under the Privacy Act. Please view our Privacy Policy at http://strategix.edu.au/documents for more information about your access rights.

Strategix will always confirm who you are before releasing to yourself or a third party. For all requests you can call our office on 07 3207 9950 and one of our friendly staff can take you through a form. Alternatively, you can complete one of the following forms, noting down details of any third parties you would like us to share your information with:

• Request for Access Form - Complete online form

OR

• Student Authority to Release Information Form - Print, sign, scan and return via email

As stated on the Request for Access Form, there is a fee associated with the retrieval of your records:

• Printed copy of Award fee: $20.00

• Copy of submitted assessments fee: $260.00

You will be prompted to make payment upon submission of the form. If payment is not received no action will be undertaken and you will not receive a response from Strategix.

Please note: There is no fee for requesting email copies of your certificates.
