Digital Form
Once you have a Vocational Placement host, you will need to complete a Vocational Placement Agreement Form by following these steps:
Get your mentor/supervisor's full name and email address
Select the appropriate link for your age group:
Fill in all the required details on the digital form and sign your part. The form will now go to the Vocational Placement host and your parent/guardian (if applicable). Once they have signed the form, you will receive a copy of the agreement form
Upload your completed Vocational Placement Agreement Form to your LMS (Cloud Assess or eLearning Portal). Your trainer will check the form and, if it is complete, will mark it as competent. Note, you will not be covered by insurance until you have been marked as competent.
The host/organisation that signs the Vocational Placement Agreement form must be the one to sign off on the VP task logs. Cannot have a different host/organisation sign off on the task logs if they didn’t sign the agreement form. If there is a change in host/organisation then a new agreement form must be signed by the new host/organisation so they can sign off on the task logs.

Alternatively, you can print this form, complete it and upload to your LMS (Cloud Assess or eLearning Portal):
