Vocational Placement Agreement Form

Digital Form

Once you have a Vocational Placement host, you will need to complete a Vocational Placement Agreement Form by following these steps:

1. Get your mentor/supervisor's full name and email address

2. Select the appropriate link for your age group:

   • Students under 18 years, select this link (Does NOT include independent students under 18 years)

   • All other students, select this link

3. Fill in all the required details on the digital form and sign your part. The form will now go to the Vocational Placement host and your parent/guardian (if applicable). Once they have signed the form, you will receive a copy of the agreement form

4. Upload your completed Vocational Placement Agreement Form to your LMS (Cloud Assess or eLearning Portal). Your trainer will check the form and, if it is complete, will mark it as competent. Note, you will not be covered by insurance until you have been marked as competent.
